Liposonix nerve healing

I had Liposonix done on my anterior thighs  4 months ago. Sometimes when I rest, I feel shooting pain in nerves, in right anterior thigh area, at random time intervals. Is this normal?

Thank you for your input after your Liposonix treatment.
Yes, this is totally normal - can do that for months after treatment. Just like with liposuction or other ultrasound devices, such as Ultherapy, there can be temporary nerve injury. Nerves tend to "let you know" when they are injured and trying to heal. They will cause those annoying twinges and tingles and shooting pains. And, it is always random (but can occur more when you are rubbing the area - like in a massage, or working out because you are stretching the nerves. I would not worry about this unless it is really painful. Nerve tenderness after Liposonix usually resolves or several months, but in some rare cases, it can take longer. Please call if you have any other concerns.

Nonsurgical treatment to tighten loose skin

I am 67 with pretty good skin and I want to tighten my skin, if possible, and my neck, with a nonsurgical treatment. I have read about Ulthera and would try this if it is effective on older skin. Please reply. Thank you, C

Thanks for your question. Ulthera can be effective on older skin. It is an ultrasound, non-invasive, tightening device and can be used on all skin types. It would be important to see you in person so that we can do a consultation to decide if it is a good option for you. I definitely believe in face-lifts and I am very honest with patients about whether they are in the right category to do a surgical procedure. Most women are ready for a face-lift between the ages of 55 and 70 (but everyone varies). At 67, you may have only mildly loose skin and could be a great candidate for Ultherapy. I am starting to recommend a series of 2 treatments to some patients. I like to space them about 6 months apart, and we bundle the price so that it is less expensive for the 2nd treatment. So, I guess the bottom line, is that yes, you might be a candidate for a non-invasive procedure. Please feel free to call for a consultation. Treatment for Lipomas

Can Thermoge be done to limpomas?

Dear K,

Thank you for your question. No Thermage can not be done to lipomas. For small lipomas, the best way to remove them is simple surgical excision. Although this does leave a small scar, it is the most successful way to treat them. Liposuction can be done for larger lipomas (bigger than a baseball). For small ones, it is almost impossible to get the liposuction cannula into the lipoma. In addition, there is a higher rate of recurrence with liposuction. In most cases, I recommend that surgical excision be done. There are no other really good treatments for lipomas. Injection with a detergent (mesotherapy) has been tried, but the success rate is very low and it often causes significant inflammation and tenderness.

Treatment for fines lines from tanning beds

Im 21, I have fine lines around my eyes and smile lines near my mouth from years of tanning in tanning beds. I also have comedonal acne. I'm looking for the best package available to help erase what I did to my skin.

Thank you for the email inquiry. Ah yes – tanning beds and too much sun can lead to early lines and aging. Of course, my first piece of advice would be to be sure that you are not tanning anymore. I hope not. I tell my patients that I have a “zero tolerance policy” for sun exposure to the face, neck, and chest. It’s the best way to stay looking young. The good news, is that if you start now while you are still young, the effects of the tanning beds will start to get better.

OK – so what can you do. 1. I would love to see you in person for a consultation so that we can look at your face and decide what the best treatment is. At your age, it may just be topical medicines (like Retin-A). However, if you have “static” lines on your face ( meaning lines that are there when you are not making an expression), then it may be the right time to start doing a little bit of Botox. That is the mainstay for wrinkles and a definite MUST to keep you looking young. We may also want to think of trying some chemical peels for you. We can address your acne as well.

Laser treatment for acne scarring

I just want to ask for the percentage of getting rid of acne and acne scars. In addition, I also want to ask for the cost of the treatments.

Thank you for the inquiry from RealSelf. The most important thing would be for you to come in for a consultation so that we can look at your acne scarring together. There are different types of scarring. Some types respond better to treatment than others. The most common treatment for acne scarring is using a laser to resurface the area and try to make the scars more even with the surrounding normal skin. Sadly, it is impossible to completely get rid of the scarring – it is, as you asked, a matter of “how much better” can we get the scarring. The two most common lasers that I use for acne scarring are the Fraxel Restore, and the Fractional CO2 laser (Total FX). I usually tell people that we are aiming for a 40% improvement. Sometimes my patients will say that they feel 80% better, and sometimes (uncommon) they say that they only feel 10% better. Some of this is that different people have different opinions on what the percentage is.
For the Fraxel Restore, it takes multiple treatments. Usually 4 to 6 treatments spaced 4 to 6 weeks apart. For cost, it depends on what we decide to do. It is usually around $2000 to $4000 total.

Filler for facial rejuvenation

Hi Dr. Reichel, I am an unhappy with my aging looks 55 year old woman ready to take some action. It is so hard looking in the mirror! My primary concern is my sagging cheeks so either Restylane or Juvederm will be in order. Eyelids depend on cost.

Filling in cheeks with a filler (usually my favorite is Radiesse, but I do a lot of Restylane and Juvederm as well) can give really nice results. It is one of my nurses favorite treatments to watch as it makes such a difference immediately. For someone with significant volume loss, I love to use fat transfer. We can talk about all of these options. We can also talk about your eyes and what types of rejuvenation might be an option – from Lasers, to fillers, to surgery. Eye rejuvenation is one of my favorites. Make sure you are using an eyecream. Belotero for tear troughs


Hi there, I'm going to be in Seattle around July 20th and wanted an appointment for fillers, particularly in the tear troughs. I've never had this before and am looking for a dr with a lot of experience in this area. Does Dr R do this? Any photos?

Thank you for your email via RealSelf. I would be happy to see you for fillers (especially in the tear troughs). I have done at least a hundred patients, if not more (specifically in that area). I love the new filler, Belotero, for this area. It is really smooth and looks super natural. There can be a few to several days of swelling and possibly bruising. Keep that in mind. How long until final result after mole removal?

Hi Dr, I was wondering how long does it take to reach the final result after mole removal? I saw pictures of mole removals on the realself website and it look like you did good work. The scars of the patient were small thin almost unrecognizable lines. I had one removed 3 months ago and it is still red and stands out. I just want it to reach its final result so I can stop thinking about it cause it bothers me and stresses me out. I wanted to know how long after were the after mole removal pictures taken? It would help ease my mind a lot thank you

Hi M,

Thank you for commenting on my “good work”. I do a lot of cosmetic mole removals and I always have a long discussion with the patient about healing time BEFORE we do the removal. It can take months for the redness and scar to resolve and look good. The photos on RealSelf that you saw were taken at 12 months and 3 years after the surgery. I comment to patients that at 2 months, you may very well be saying “why did I do this, it looks worse than the mole”. BUT, let me reassure you. In 90% of cases, the redness clears and the scar improves greatly and the cosmetic result is nice. It can take 4-12 months for that good end-point. So I hope this eases your mind.

I would give it another month, and then if the redness is still present, or the scar is not looking good, I would recommend that you 1. Talk to the doctor that removed the mole and see if they will see you – it is always good to let the physician that did the work know how things are going. He/She may have a laser or other options to improve scars. After you do that, you can 2. Always make an appointment here at Pacific Dermatology & Cosmetic Center and I would be happy to look at the scar and see if we can improve it, or at least reassure you that it will get better. I have many lasers (Vbeam, Fraxel, CO2) that can improve scars.
